Job summary

Oak Street Health in Waterbury, CT is seeking a Medical Scribe to support primary care providers with real-time documentation in clinical encounters. This full-time, on-site position offers hands-on experience with CPT/ICD-10 coding and after-visit summaries.

You will observe patient visits in the exam room, document history, assessment and plan, and collaborate with clinicians to ensure accurate, timely records. Strong communication, fast typing, and willingness to learn are essential.

Qualifications

• Knowledge of medical terminology and common medications
• Prior clinical experience or shadowing in healthcare settings preferred
• Scribe or transcription experience preferred but not required

Responsibilities

• Document patient encounters in real time with structured notes
• Assign CPT and ICD-10 codes for encounters
• Prepare After Visit Summaries
• Support clinical documentation improvement and data accuracy
• Provide administrative support to the care team
