Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time, preparing visit summaries, and coding CPT/ICD-10 entries accurately. You will observe visits, record histories, assessments, plans, and exams, and collaborate with the care team to close documentation gaps.

This role offers paid, hands-on experience in a clinical setting. Ideal candidates commit to 1–2 years, have medical terminology knowledge, and are fluent in

Qualifications

• Knowledge of medical terminology and common medications required.
• Prior clinical experience, including shadowing and/or volunteering strongly preferred.
• Prior scribe or transcription experience preferred but not required.

Responsibilities

• Documenting patient encounters in real time with a structured note, including history, assessment, plan, and exam.
• Assigning CPT and ICD-10 codes.
• Preparing After Visit Summaries.
• Supporting daily huddles and clinical documentation reviews.
• Administrative support for the care team, including orders and referrals.
• Other duties as assigned.
