Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time, in the exam room, and preparing after-visit summaries.

Ideal candidates commit to 1–2 years, bring knowledge of medical terminology, strong listening and computer skills, and are open to learning in a fast-paced clinical setting with full-time hours and comprehensive benefits.

Competenze

• Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience [required]
• Prior clinical experience, including shadowing and/or volunteering [strongly preferred]
• Prior scribe or transcription experience [preferred but not required]

Mansioni

• Document patient encounters and input accurate notes.
• Observe patient visits in the exam room with the provider.
• Document encounters in structured notes including HPI, assessment, plan and exam.
• Assign CPT and ICD-10 codes.
• Prepare after-visit summaries.
• Consult with provider to ensure accurate documentation.
• Assist with clinical documentation improvement.
• Request and review medical records.
• Leverage population health tools to support documentation improvement.
• Support daily huddles and clinical documentation reviews.
• Consult with provider on documentation opportunities.
• Provide administrative support for the provider and care team.
• Place orders and referrals.
• Address tasks as assigned.
• Support the care team with additional documentation responsibilities.
• Other duties as assigned.
