Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time clinical documentation. You will observe visits, document encounters in structured notes (history, assessment, plan, exam), assign CPT/ICD-10 codes, and participate in daily huddles and after-visit summaries.

Ideal candidates have 1–2 years in a clinical setting, strong typing skills (70+ WPM), and knowledge of medical terminology.

Qualifications

• Medical terminology and medications knowledge required.
• Prior clinical shadowing or volunteering preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in structured notes (HPI, assessment, plan, exam).
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Consult with provider to ensure accurate documentation.
• Support clinical documentation improvement and daily huddles.
• Place orders and referrals as needed.
• Other duties as assigned.
