Job summary

Oak Street Health seeks a Medical Scribe to support primary care providers by documenting patient encounters in real time, including history, assessment, plan, and exam.

Scribes learn value-based care processes and CPT/ICD coding, gaining experience with chronic conditions and population health data. This full-time role requires strong communication, computer literacy, and a commitment of 1–2 years.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience (shadowing/volunteering) preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Documenting patient encounters in real time (history, assessment, plan, and exam).
• Assigning CPT and ICD-10 codes and preparing after-visit summaries.
• Clinical documentation improvement and reviewing patient records.
• Administrative support for the provider and care team; other duties as assigned.
