Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time and ensuring precise, timely clinical documentation. You will observe visits, enter structured notes, assign CPT/ICD-10 codes, and help prepare after-visit summaries while collaborating with the care team.

Ideal candidates have medical terminology knowledge and clinical exposure, strong listening and communication skills, and the ability to adapt to new workflows.

Qualifications

• Knowledge of medical terminology and common medications from pre-medical studies or clinical experience.
• Prior clinical experience, including shadowing or volunteering.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in structured notes with HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Support clinical documentation improvement with records review.
• Administrative support for the care team.
