Job summary

Oak Street Health is seeking a Medical Scribe to support clinicians with real-time documentation during patient visits in our clinics. You will observe encounters with providers, document histories, assessments and plans, and assign CPT/ICD-10 codes.

You’ll also prepare After Visit Summaries and assist with daily huddles and documentation improvement to ensure accurate records for value-based care. Strong communication, medical terminology knowledge, and the ability to type quickly are essential

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Prior scribe or transcription experience.

Responsibilities

• Documenting patient encounters in a structured note, including history of present illness, assessment, plan and physical exam.
• Assign CPT and ICD-10 codes.
• Preparing After Visit Summaries.
• Clinical Documentation Improvement: review medical records and support documentation opportunities.
• Administrative support: placing orders and referrals, addressing tasks for the care team.
