Job summary

Oak Street Health in Cleveland is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time and assisting with clinical workflows. You will observe visits, assign CPT/ICD-10 codes, prepare After Visit Summaries, and contribute to documentation quality within our value‑based care model.

This full‑time role offers training, competitive pay, and benefits. Ideal candidates have medical terminology knowledge, prior clinical exposure, and strong typing

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Documenting patient encounters in real time.
• Join the provider in the exam room to observe patient visits.
• Document patient encounters in structured notes with HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Consult with provider for accurate documentation.
• Clinical Documentation Improvement – contribute to coding accuracy.
• Review medical records as needed.
• Leverage population health tools to support documentation improvement.
• Support daily huddles and Clinical Documentation Reviews.
• Assist with placing orders and referrals.
• Provide administrative support to the care team.
