Job summary

Oak Street Health seeks a Medical Scribe to support primary care providers with real-time clinical documentation, enabling focused patient care. You will observe in exam rooms, document encounters, and help with CPT/ICD-10 coding and after-visit summaries.

Ideal candidates have strong listening skills, good computer literacy, and potential bilingual abilities; commitment of 1 year minimum is preferred, with full-time scheduling on-site.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Prior scribe or transcription experience.

Responsibilities

• Documenting patient encounters ~ 80% of the role.
• Joining the provider in the exam room to observe patient visits.
• Documenting patient encounters in structured notes with HPI, assessment, plan, and exam.
• Assigning CPT and ICD-10 codes.
• Preparing After Visit Summaries.
• Consulting with provider to ensure accurate documentation.
• Clinical documentation improvement tasks and reviewing medical records.
• Supporting daily huddles and clinical documentation reviews.
• Administrative support for the provider and care team, including placing orders and referrals.
