Job summary

Oak Street Health is hiring a Medical Scribe to support primary care providers with clinical documentation. You will observe visits, document encounters, and help with CPT/ICD-10 coding, ensuring accurate, timely notes.

Qualifications

• Listening and communication abilities are required.
• Strong computer literacy and ability to learn new workflows.
• Fluency in Spanish, Polish, Russian, or other languages common in the communities served.

Responsibilities

• Documenting patient encounters in a structured note including HPI, assessment, plan, and exam.
• Join the provider in the exam room to observe patient visits.
• Assign CPT and ICD-10 codes accurately.
• Prepare After Visit Summaries.
• Support clinical documentation improvement within a value-based care model.
• Coordinate with providers to ensure timely documentation.
