Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time documentation and coding accuracy. The role involves observing visits, documenting encounters, and helping prepare after-visit summaries to ensure high-quality patient care.

Ideal candidates have strong listening and communication skills, are computer-literate, and can adapt to evolving clinical workflows. Open to bilingual or multilingual candidates to serve diverse communities.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience or shadowing.
• Scribe or transcription experience is a plus.

Responsibilities

• Document patient encounters in a structured note with history, assessment, plan and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries and support care team with documentation.
• Support clinical documentation improvements and reviews.
