Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time documentation and to observe visits in the exam room. You will help with CPT and ICD-10 coding, prepare After Visit Summaries, and collaborate with the care team to ensure accurate clinical documentation.

This role focuses on contributing to value-based care and timely patient notes. Ideal candidates have medical terminology knowledge, some clinical exposure, and strong communication skills.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience preferred.
• Scribe or transcription experience optional.

Responsibilities

• Documenting patient encounters in real time with structured notes (history, assessment, plan, exam).
• Assigning CPT and ICD-10 codes.
• Prepare After Visit Summaries and support care team with documentation reviews.
• Support daily huddles and clinical documentation reviews.
• Provide administrative support to provider and care team, including orders and referrals.
