Job summary

Grady Health System in Atlanta seeks a Clinical Care Coordinator (C3). The C3 proactively identifies barriers to patient flow and serves as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources.

The HUB C3 maintains a system-wide view of hospital operations, coordinating admission, placement, flow, staffing and procedures. The HUB C3 collaborates with nursing units and service departments to ensure appropriate

Qualifications

• Bachelor's degree in a health-related field required
• Three (3) or more years of clinical experience required
• Registered Nurse in the State of Georgia and Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) for Critical Care and ECC units

Responsibilities

• Identify and remove barriers to department or system-wide patient flow; act as consultant and educator with medical staff on patient status, bed placement, payor requirements and resource utilization.
• Serve as primary liaison between all service locations, support departments, and physicians to ensure appropriate patient placement and efficient progression of care.
• Collaborate with the healthcare team to improve length of stay (LOS) and patient-focused outcomes across Grady Health System.
