Job summary

Solü Technology Partners in Phoenix, AZ seeks a patient-focused on-site support specialist to assist members with education, scheduling and referrals, ensuring timely care and positive outcomes. This role emphasizes empathetic patient service and collaboration with the Care Team to manage referrals and follow-up.

You will coordinate end-to-end referral processes, support members with chronic conditions through reminders and education, and participate in quality programs to improve health

Qualifications

• CPR certification required.
• Excellent interpersonal and communication skills.
• Experience with Electronic Medical Records (EMR).
• Customer service orientation and experience.
• Must work on-site; 100% on-site and may move between clinics.
• AZ fingerprint clearance card required.
• Ability to work mornings, nights, or weekends.
• Bilingual English/Mandarin preferred but not required.

Responsibilities

• Support members in-person and virtually with real-time education and scheduling of specialist and diagnostic referrals.
• Coordinate follow-up and ensure completion of appointments and referrals.
• Improve health outcomes and contribute to cost savings.
• Be an empathetic problem-solver and serve as a visible partner to patients.
• Assist in development and management of quality programs including patient outreach for gaps in care.
• Support members with chronic conditions through education, reminders, and other support.
• Lead service and schedule requests on behalf of members.
• Inform Care Team with referral information and requirements.
• Assist in specialist relationship management.
• Perform other duties as assigned.
• Work hours include morning, night, or weekend hours.
• This position requires 100% on-site work at the care center location and may involve movement between clinics.
