Job summary

Included Health is seeking a Referral Support Advocate to join our Care Team in delivering world‑class provider referrals for members, coordinating both in‑person and virtual visits, and guiding members through their health care journey.

You will demonstrate clear verbal and written communication, meticulous data collection, and strong organizational skills while collaborating with Care, Records, Clinical, Quality, and Training teams.

Qualifications

• Excellent verbal communication, ability to explain services to providers and verify availability.
• Strong written communication to share updates with the care team.
• Assertive in asking questions; timely escalation when needed.
• Detail oriented; accurate data collection and entry.
• Adaptability in fast-paced, changing environments.
• Proficient in internet searches for provider information.
• Able to troubleshoot basic technical issues.
• Proficient with Google Apps and CRM systems.
• Strong organizational skills and time management.
• Sense of humor and teamwork mindset.

Responsibilities

• Understand and complete all Concierge Referrals tasks and present high-quality providers.
• Coordinate with scheduling teams to arrange appointments for members.
• Balance speed and quality to delight members.
• Meet or exceed service level agreements.
• Gather accurate data from provider offices including availability and contracts.
• Work in a fast-paced environment with rapid changes.
• Communicate clearly within Care, Records, Clinical, Quality teams.
• Drive continuous improvement in processes and member experience.
• Grow in role and develop subject matter expertise.
• Discuss additional Included Health benefits with relevant departments.
