Community Living in Yakima, WA seeks a dedicated professional to coordinate client medical care and manage health-related records. You will support direct care staff, schedule medical appointments, and ensure documentation is accurate and up to date.

The role includes PCSP/IISP planning, training DSPs on medications and hygiene, and providing urgent 24/7 support while maintaining safety and quality of life for clients.

Qualifications

• High school diploma or GED equivalent.
• At least 18 years of age.
• Proof of eligibility to work in the United States.
• Possess a valid driver’s license.
• Possess an insured and safe vehicle.
• Must be able to read, write, speak and understand English.
• Ability to pass a DSHS background check.
• Satisfactory motor vehicle report.
• Ability to work with minimal supervision.
• Ability to work effectively under high stress/pressure situations.
• Ability to work a flexible schedule, which may include early mornings, late evenings and weekends.
• Ability to initiate independent and appropriate judgement and decisions.

Responsabilités

• Coordinate client medical care with staff and guardians.
• Manage medical files in SharePoint and hard copies.
• Schedule client appointments and transport.
• Provide health updates to guardians and staff.
• Train DSPs on medication programs and hygiene.
• Review PCSP/IISP documents for accuracy.
• Maintain client files to meet policy requirements.
• Provide 24-hour emergency support to clients.
• Follow safety guidelines and document health changes.
