Job summary

Volunteers of America Western Washington seeks a Client Care Manager to develop and implement client care plans, coordinate medical appointments and medications, and support overall health care needs. You will lead transitions, maintain records, and ensure continuity of care across care teams.

The role includes onboarding and training new caregivers, monthly documentation reviews, and participation in annual assessments. Some travel and driving may be required.

Qualifications

  • Bachelor’s degree in a human services field and at least 1 year experience in a case management or health care setting.
  • Basic understanding of developmental diagnoses and nutrition related health care needs.
  • Familiarity with case management and medication management, Nurse Delegation laws.
  • Proficient in computer operation, with basic knowledge of Microsoft Office Suite.
  • Strong organizational and communication skills.

Responsibilities

  • Partner with clients, guardians, and support teams to create and implement care plans aligned with the Person-Centered Service Plan.
  • Coordinate medical appointments, medications, and ensure continuity of care.
  • Maintain client records and releases of information; schedule and attend healthcare appointments.
  • Train new employees and provide client-specific training as needed.
  • Attend annual assessments and participate in care team coordination; review documentation monthly.

This role creates plans in alignment with WAC, DDA, and internal policies and procedures. This position involves coordinating and monitoring clients' medical appointments, prescribed medications, and supporting overall health care needs. They support clients through transitions, coordinating with the care teams to ensure continuity of care and minimize disruptions. This position also oversees the hiring and onboarding of new caregivers.Principal ActivitiesPartner with clients, guardians, and support teams to create and implement all care plans that align with their Person-Centered Service Plan.Obtain and maintain all releases of information.Communicates with medical and insurance providers to facilitate coordinated care.Responsible for the organization and maintenance of all client records.Coordinates with team to ensure healthcare appointments are scheduled and attended.Collaborates with Nurse Delegator to ensure they have received communication from teams regarding any changes to delegated tasks and medication.Meets with clients and DSPs to discuss client progress toward goals and incorporates any new information or goals into client support plans.Works together with the training department to coordinate the initial training of new employees and any client specific training needs.Conduct monthly review of client documentation including MARs and charting, and coordinates with the Program Manager to address any discrepancies.Attends all annual assessments as part of the client’s care team.Attend medical appointments as needed.Perform additional duties as assigned.BenefitsWe believe that taking care of our team is essential to fulfilling our mission.
