Job summary

Qualifications

• Proficiency with EHR and Microsoft Office products.

Responsibilities

• Provide day-to-day supervision, leadership, coaching, mentoring, and performance management for Community Health Workers and staff.
• Recruit, orient, train, and support staff development and professional growth.
• Oversee completion of patient referrals and follow-through of service delivery and documentation.
• Ensure compliance with funding contracts, quality standards, and reporting.
• Develop and maintain collaborative relationships with community partners and providers.
