Yakima Neighborhood Health Services is seeking a community-oriented team member to assist patients and community members in learning about and applying for resources, including state and federal assistance programs. The role emphasizes coordination with navigators to connect individuals to health coverage, housing, and support services.

The ideal candidate will have strong data-entry and communication skills, preferred bilingual English/Spanish abilities, and a demonstrated commitment to serving

Qualifications

• Knowledge of community services, social services, and community resources required.
• Working command of spelling and punctuation required.
• Skills in data-entry.
• Ability to speak clearly and concisely.
• Preferred bilingual English/Spanish; ASL sign language helpful.

Responsibilities

• Assist patients and community members in learning about and applying for resources and programs.
• Engage with programs like TANF, SNAP, WIC, Apple Health, ECEAP, and WIOA.
• Work with Navigators to connect clients to health coverage and housing services.
• Document contacts and interactions in the Electronic Health Records system.
• Participate in outreach to serve low-income populations and diverse communities.
