Responsibilities

• Support the Client Engagement Manager to help clients develop and support their reentry plan.
• Utilize technology to track and report activities and results of participants.
• Filing and maintaining client case history reports.
• Interview clients to assess situational needs and determine necessary services.
• Serve as liaison between client and social, financial, health and legal services.
• Develop action plans to improve client well-being and plan follow-ups.
• Reviewing and adjusting plans as circumstances change for individuals.
