Job summary

Televerde Foundation is seeking an Engagement Specialist to develop and maintain engagement and participation for clients throughout the program lifecycle. Qualifications

• 3-5 years of case management, adult education, or nonprofit program management
• Experience scheduling, managing and tracking programs or activities
• Ability to clear Department of Corrections visitor background process
• Compassionate and able to relate to clients with various needs
• Strong verbal and written communication skills to explain the case and care plan and maintain records
• Motivation to encourage clients to follow their care plans
• Flexibility to change care plans if they are not getting the best results
• Organization to manage several cases at once
• Computer literacy to maintain and manage case records

Responsibilities

• Support the Client Engagement Manager to assist with clients to develop and support their reentry plan
• Utilize technology to track and report on the activities and results of participants
• Filing and maintaining case history reports
• Interviewing clients to assess their situational needs
• Determining which services and assistance are necessary
• Serving as a liaison between the client and social, financial, health and legal services
• Developing a plan of action to improve the client’s well-being
• Planning regular follow-ups to assess progress and additional needs
• Reviewing and modifying plans as circumstances change for the individual

Education

Advanced education or training in related field or equivalent experience
