Televerde in Phoenix, AZ is seeking an Engagement Specialist to develop and maintain client engagement throughout the program lifecycle. The role focuses on coordinating care plans, tracking participant activity, and preparing case histories.

You will support the Client Engagement Manager, use technology to monitor progress, and ensure services meet client needs. This is a full-time position based in an office environment.

Responsabilidades

• Serving as a liaison between the client and social, financial, health and legal services
• Developing a plan of action to improve the client’s well-being
• Planning regular follow-ups to assess progress and additional needs
• Reviewing and modifying plans as circumstances change for the individual

Salary Range: $45,000.00 To $52,500.00 Annually

TheEngagement Specialistis responsible for developing and maintaining engagement and participation of the client throughout the entire program lifecycle.

Duties and responsibilities

• Support the Client Engagement Manager to assist with clients to develop and support their reentry plan

Qualifications

• Ability to clear Department of Corrections visitor background process
• Compassionate and able to relate to different clients with various needs
• Motivational to encourage clients to follow their care plans
• Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records
• Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers
• Flexibility to change care plans if they are not getting the best results
• Organization to manage several different cases at once
• Computer literacy to maintain and manage case records.

Education/ Experience

• Advanced educations or training in related field or equivalent experience preferred
• 3-5 years of case management, adult education, or non-profit program management
• Experience scheduling/managing and tracking programs or activities

Working conditions/ Physical requirements

This position is performed in an office environment.

Essential physical requirements of this job include:

• Ability to move wrists, hands, and/or fingers in a repetitive motion.
• Ability to continuously stand, walk, reach, kneel, bend, stoop, push/pull, and sit.
• Ability to lift 25 pounds occasionally.
• Communicating effectively with employees, vendors, and clients.
• Continuously working at a desk and computer screen at least 50% of the time.
