Job summary

Legacy at Town Creek in Palestine, TX is seeking a motivated Registered Nurse or Licensed Practical Nurse to join its memory care team.

This role focuses on coordinating with the Medical Director and attending physician, evaluating residents, and supporting the DON and clinical staff to provide exceptional long-term care.

Qualifications

• Current RN or LPN license in good standing with the State Board of Nursing.
• Ability to work 12-hour shifts (6a-6p or 6p-6a).
• Desire to grow professionally under guidance of DON/ADON and staff.

Responsibilities

We are opening our Memory Care Center for long-term senior care and are in search of knowledgeable staff with the heart to help us provide this much needed care. We are growing our team and adding Amazing Healthcare Professionals. What we need to get there is a motivated Registered Nurse or Licensed Practical Nurse who wants to be part of a great team and to be committed to growing her/his talents under the guidance of a supportive DON, knowledgeable ADON, and great clinical staff.

What You’ll be Doing

• Conferring with the Medical Director and the attending physician regarding specific residents assigned to you.
• Consulting with the physician concerning resident evaluation and assist the Director of Nursing in planning and developing the nursing services to be performed for the resident.
• Helping your team members grow their careers—you will change lives!
• Examining residents and their records and charts to distinguish between normal and abnormal findings in order to recognize early stages of serious physical, emotional or mental problems.
• Performing administrative duties regarding assigned residents, such as completing medical forms, reports, evaluations, studies, charting, etc.
