Job summary

Priority Management Group, LLC in Castroville, TX, is seeking a motivated LPN/LVN to join our nursing team at Medina Valley Health and Rehabilitation Center. The role focuses on resident care, collaboration with the Medical Director and physician, and maintaining detailed charts.

You will work Mon-Fri 2pm-10pm with a rotating 10pm-6am shift option, supporting the DON and ADON, and helping residents achieve optimal health. A current LPN/LVN license in good standing is required.

Qualifications

• Must have a current LPN/LVN license in good standing with the State Board of Nursing.
• Experience in long-term care is preferred.
• Strong teamwork and communication skills.

Responsibilities

• Conferring with the Medical Director and the attending physician regarding specific residents assigned to you.
• Consulting with the physician concerning resident evaluation and assist the Director of Nursing in planning and developing the nursing services to be performed for the resident.
• Helping your team members grow their careers- you will change lives!
• Examining residents and their records and charts to distinguish between normal and abnormal findings in order to recognize early stages of serious physical, emotional or mental problems.
• Performing administrative duties regarding assigned residents, such as completing medical forms, reports, evaluations, studies, charting, etc.

Position Open: Mon-Friday 2pm-10pm X2- (NURSING AND MED.AIDE) 10pm-6a Rotating schedule(4 nights on-2 days off)
