Elite Care Management, Inc. is seeking a compassionate Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to join our private duty/home health care team in Lake Placid, FL.

This role involves caring for one patient per shift, building a strong relationship with the patient and family, and supporting daily living activities to enhance quality of life. You will perform ADLs, monitor vitals, provide comfort, and collaborate with the supervisor to ensure excellent patient outcomes.

Qualifications

• Florida State CNA or HHA certification (75‑hour program) is required.
• Current CPR certification is required.
• Social Security Card, Driver’s License and Auto Insurance Card are required.
• TB Test (2‑step within last 12 months or QuantiFERON within 5 years) is required.

Responsibilities

• Provide care to ONE patient for the full shift, building rapport with patient and family.
• Assist with ADLs (bathing, dressing, grooming, eating) and mobility.
• Monitor patient vitals (pulse, temperature, respiration).
• Record patient care and report observations to supervisor/case manager.
• Perform light housekeeping, meal prep, and ensure patient comfort and safety.
• Engage patient to maintain mental alertness and quality of life.

Responsibilities

Elite Care Management, Inc. is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our dynamic, growing home healthcare team! We have been a leading provider over 25 years in the private duty/home health care industry, we pride ourselves on delivering exceptional patient care and improving health outcomes within our community.

You will work with ONE PATIENT for your complete shift, getting to know the patient, their needs, and their family life. As a skilled CNA or HHA, you will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager.
• Support mobility by assisting patients with lifting and transferring as needed.
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
• Light house-keeping duties, such as changing bed linens, laundry, cleaning, or preparing meals.
• Enhancing quality of life and building relationships with patients through conversation, doing activities together, and finding stimulation to keep them mentally healthy and alert.
• Monitor patients’ pulse, temperature, and respiration.
• FL State CNA Certification or HHA certificate from a 75 hour training program
• Current CPR Certification
