Job summary

Elite Care Management, Inc. is seeking a compassionate CNA to join our home health care team for a patient-focused, single-patient care shift in Lake Placid, FL.

The role involves assisting with daily living activities, monitoring health, and building meaningful interactions with the patient. You will document care, support mobility, and maintain the patient’s comfort and dignity while providing essential companionship.

Qualifications

• Compassionate, dedicated CNA/HHA with patient-centered approach.
• Experience in home health care and daily living assistance is preferred.
• Ability to maintain accurate patient records and report observations promptly.
• CPR certification is required.

Responsibilities

• Maintain records of patient care, conditions, progress or problems.
• Support mobility by assisting patients with lifting and transferring.
• Assist patients with ADLs such as bathing, dressing, grooming and eating.
• Light housekeeping duties including changing bed linens, laundry, cleaning, or meals.

Responsibilities

Elite Care Management, Inc. is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our dynamic, growing home healthcare team! We have been a leading provider over 25 years in the private duty/home health care industry, we pride ourselves on delivering exceptional patient care and improving health outcomes within our community.

You will work with ONE PATIENT for your complete shift, getting to know the patient, their needs, and their family life. As a skilled CNA or HHA, you will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager.
• Support mobility by assisting patients with lifting and transferring as needed.
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating.
• Light house-keeping duties, such as changing bed linens, laundry, cleaning, or preparing meals.
• Enhancing quality of life and building relationships with patients through conversation, doing activities together, and finding stimulation to keep them mentally healthy and alert.
• Monitor patients’ pulse, temperature, and respiration.
• FL State CNA Certification or HHA certificate from a 75 hour training program
• Current CPR Certification
• Social Security Card
• Driver’s License and Auto Insurance Card
• TB Test (2-step within the last 12 months OR QuantiFERON within 5 years)
