Moments Hospice in Wisconsin is seeking a dedicated CNA to provide compassionate personal care to patients in their homes. This on-the-road role includes a company car with gas and insurance covered, a cell phone, and on-call support, enabling you to deliver comfort and dignity during patients' final days.

You'll work under a Case Manager RN, review care plans, assist with transfers, mobility, nutrition, and maintain electronic medical records, all while building meaningful relationships with

Qualifikationen

• Must hold a CNA license and have at least 12 months of caregiving experience.
• Current CPR certification preferred.
• Excellent customer service, strong communication, and problem-solving skills.
• Ability to lift 50 lbs. and understand body mechanics.
• Valid driver's license and auto insurance required.
• Experience with electronic medical records (EMR).

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
