Job summary

Moments Hospice is seeking a dedicated Certified Nursing Assistant (CNA) to provide compassionate daily living support for patients at home or in care settings. You will work under a Case Manager RN, perform personal care tasks, review plans, and assist with mobility and nutrition while documenting care in the EMR.

Qualifications

Certified Nursing Assistant (CNA) license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
Current CPR certification is preferred.
Excellent customer service skills and strong communication abilities.
Ability to work in an electronic medical record system.
Must be able to lift up to 50 lbs.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
