Job summary

Moments Hospice is seeking a compassionate CNA to provide daily living assistance and patient care in a home-based setting. You will work under the direction of a Case Manager RN, review care plans, and help with mobility, nutrition, and records management.

Qualifications

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN
