Moments Hospice is seeking a CNA to join our team on the road, providing essential personal care and support to patients in their final days. You will collaborate with an RN Case Manager, perform daily living activities, and maintain patient records with a focus on comfort and dignity.

Qualifying candidates hold a CNA license, have at least 12 months of caregiving experience, and may have CPR certification.

Formación

• CNA license is required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required.
• Current CPR certification is preferred.

Responsabilidades

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)
