Job summary

Moments Hospice is seeking a compassionate CNA to provide direct patient care on the road, supporting comfort and dignity for patients in their final days. You will work closely with an RN Case Manager, delivering personal care, documenting in EMR, and ensuring safe transfers.

The role emphasizes teamwork, strong communication, and adherence to care plans, with benefits including a company car, health coverage, and PTO. Requires CNA license and at least 12 months’ caregiving experience.

Qualifications

CNA license is required.
At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
Current CPR certification preferred.
Excellent customer service skills.
Strong written and verbal communication and active listening.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
Review and follow patient care plans with RN Case Manager.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records (EMR).
Work under the direction of a Case Manager RN.
