Moments Hospice is seeking a compassionate CNA to provide personal care and support to patients at the end of life. You will work under a Case Manager RN, review care plans, and collaborate with an RN team to ensure comfort and dignity.

The position includes a company car option with fuel and insurance coverage, a cell phone, competitive pay, and comprehensive benefits. On-the-road duties require strong caregiving skills and reliable patient-focused care.

Formación

• CNA license required.
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs and maintain body mechanics.
• Understanding of professional boundaries.
• Valid driver's license required.
• Valid Auto Insurance required.

Responsabilidades

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with ROM, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
