Job summary

Moments Hospice is seeking a dedicated CNA to provide compassionate patient care on the road. The role includes personal care activities, reviewing care plans, and assisting with transfers under an RN Case Manager supervision.

Qualifications

CNA license is required.
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
CPR certification is preferred.
Excellent customer service skills are required.
Strong communication, written and verbal skills are required.
Able to lift 50 lbs. and understand body mechanics.
Valid driver's license and auto insurance are required.

Responsibilities

Perform daily living activities (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
