Job summary

Moments Hospice seeks a compassionate Certified Nursing Assistant (CNA) to support patients in daily living activities and comfort during their final days. You will collaborate with a dedicated RN Case Manager, perform personal care, review care plans, and maintain electronic medical records, all while upholding dignity and high standards of care.

Qualifications

• CNA license is required.
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license required.
• Valid Auto Insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
