Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to support patients in their homes across the service area. The role involves direct patient care, collaboration with a Case Manager RN, and maintaining accurate electronic medical records.

You will work on the road, providing essential daily living assistance, with on-call support and structured training to advance your skills. A company car with fuel and insurance, cell phone, and comprehensive benefits are included.

Formación

CNA license is required.
At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
Current CPR certification is preferred.

Responsabilidades

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
