Job summary

Moments Hospice in Minnesota is seeking a compassionate CNA to provide personal care and support under the guidance of an RN Case Manager. This role emphasizes patient comfort, dignity, and a meaningful contribution to end-of-life care, with a schedule that suits on-the-road responsibilities.

Qualifications

CNA license required and active.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
Current CPR certification (preferred).
Excellent customer service skills.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications:

Certified Nursing Assistant (CNA) license (required)
