Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to join our team. You will provide hands-on personal care to patients at their homes, assisted living facilities, or care facilities, helping them stay comfortable and dignified in the final chapter of life.

You will collaborate with a dedicated RN Case Manager and be part of an interdisciplinary team with structured training and growth opportunities.

Qualifications

• Active Certified Nursing Assistant (CNA) certification is required.
• Experience in home care, assisted living, or hospice is a plus.
• Current CPR certification preferred.
• Ability to use electronic medical records.
• Strong communication and time management skills.
• Must lift up to 50 lbs and have good body mechanics.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
• Assist with dining and nutrition
• Review and follow patient’s plan of care under the direction of the RN Case Manager
• Provide comfort and companionship to patients and support to their families
• Maintain electronic medical records

Certified Nursing Assistant (CNA) / Hospice Aide

Moments Hospice

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
