Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care for patients in private homes, assisted living, or care facilities. You will work with an RN Case Manager and a supportive interdisciplinary team to ensure comfort, dignity, and compassionate end-of-life care.

This full-time field-based role offers a pay range of $18–$23 per hour, structured schedule, and benefits including fleet car use, company phone, and comprehensive insurance

Qualifications

• Active Certified Nursing Assistant (CNA) certification is required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice is a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical record system.
• Strong communication skills, including written, verbal, and active listening.
• Excellent customer service, problem-solving, and time management.
• Compassion, patience, and the ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs and understanding of body mechanics.
• Valid driver''s license and current auto insurance required.

Responsibilities

• Provide personal care with activities of daily living (bath, dress, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient''s plan of care under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
