Job summary

Moments Hospice in Fergus Falls, MN is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide compassionate, hands-on personal care in patients' homes, assisted living, or care facilities. You will work with an RN Case Manager as part of an interdisciplinary care team.

The role is full-time and field-based with hours Monday–Friday, 8:30 AM – 5:00 PM. Pay ranges from $18 to $23 per hour, with a fleet car program and a comprehensive benefits package including medical, dental,

Qualifications

• Active CNA certification required.
• Healthcare caregiving experience in home/home care or hospice is a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical record system.
• Strong communication and time-management skills.
• Compassion, patience and ability to handle emotional situations.

Responsibilities

• Provide ADL care: bathing, dressing, grooming, toileting and skin care.
• Assist with lifting, transferring, positioning and ambulation.
• Assist with dining and nutrition.
• Follow patient care plans under RN supervision.
• Provide comfort and companionship to patients and families.
• Maintain electronic medical records.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
• Assist with dining and nutrition
• Review and follow each patient’s plan of care under the direction of the RN Case Manager
• Provide comfort and companionship to patients and support to their families
• Maintain electronic medical records

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
