Job summary
Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to support patients in their homes across the service area. The role involves direct patient care, collaboration with a Case Manager RN, and maintaining accurate electronic medical records.
You will work on the road, providing essential daily living assistance, with on-call support and structured training to advance your skills. A company car with fuel and insurance, cell phone, and comprehensive benefits are included.
Qualifications
- CNA license is required.
- At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
- Current CPR certification is preferred.
Responsibilities
- Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
- Review patient care plans.
- Assist with lifting and transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition.
- Maintain electronic medical records.
- Work under the direction of a Case Manager RN.
