Job summary

Moments Hospice in Minnesota is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care in patients' homes or care facilities. You will work with an RN Case Manager, follow plans of care, and support families.

Qualifications

• Active CNA certification is required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice is a plus.
• Current CPR certification is preferred.
• Ability to work in an electronic medical record system.
• Strong communication, written and verbal skills needed.
• Excellent customer service, problem-solving and time management abilities.
• Compassion and patience, with capacity to handle emotional situations with care.
• Understanding of professional boundaries and ability to lift 50 lbs.

Responsibilities

• Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, ROM, exercises and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
