Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care for patients in home, assisted living, or care facilities. You will work with a dedicated RN Case Manager within an interdisciplinary team, focusing on comfort, dignity, and compassionate care in the final chapter of life.

The role includes a fleet car program, company cell phone, and comprehensive benefits.

Qualifikationen

• Active CNA certification (required).
• Healthcare caregiving experience in home care, assisted living, or hospice a plus.
• Current CPR certification (preferred).
• Ability to work in an electronic medical record system.
• Strong communication skills, including written and verbal.
• Compassion, patience, and ability to handle emotional situations with care.
• Ability to lift 50 lbs and maintain safe body mechanics.

Aufgaben

• Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, ROM, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Provide comfort and companionship to patients and support to their families

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
