Apple Rehab Inc. is seeking a Centralized Admissions Coordinator to support hospital partnerships and streamline admissions for residents in our skilled nursing facilities. The role focuses on reviewing referrals, collaborating with hospital case managers, and ensuring timely, compliant admissions.

The position requires strong communication, attention to detail, and a team-oriented mindset. It is a full-time, on-site role based in Avon, CT with Monday–Friday hours, rotating holidays, and weekend

Qualifications

• Excellent communication and problem-solving abilities.
• Strong attention to detail and ability to prioritize tasks.
• Experience in healthcare, case management, admissions, or related field preferred.

Responsabilités

• Review referrals from hospitals for skilled nursing facilities.
• Collaborate with hospital case managers and internal teams for timely admissions.
• Make clinical and financial admission decisions to guide patient care level.
• Plan, organize, and execute the admission process in compliance with regulations.
• Be part of a high-performing team with a patient-first approach.
