Accela Healthcare is seeking an Admissions Coordinator for a skilled nursing and rehabilitation facility in Hamilton, NJ. The role focuses on welcoming new residents, coordinating the admission process from referral to arrival, and collaborating with hospitals, physicians, social workers, and families to ensure efficient admissions.

The position is full-time with Tuesday-Saturday shifts (9A-5P or 10A-6P). The Admissions Coordinator will work closely with clinical teams to support census goals

Qualifications

• Experience in skilled nursing or post-acute care settings is preferred.
• Strong communication and interpersonal skills are essential.
• Excellent organization and attention to detail are required.

Responsibilities

• Coordinate and facilitate admissions for new residents.
• Review referrals from hospitals, physicians, and other partners.
• Collect and verify demographic, insurance, and admission information.
• Coordinate required documentation prior to admission.
• Communicate with hospitals, discharge planners, social workers, and families.
• Ensure information is conveyed to nursing and clinical teams before admission.
• Assist residents and families with questions during the admission process.
• Manage admission logistics and smooth transitions into the facility.
• Maintain accurate admission records and downturn documentation.
