This full-time role supports a fast-paced environment with attention to accuracy and compassionate service. Responsibilities include coordinating with hospital partners, ensuring timely admissions, and maintaining compliance with all regulatory guidelines.

We’re looking for a motivated, detail-oriented, and compassionate individual to join our Centralized Admissions Team, supporting hospital partnerships and helping patients access the care they need.

Position:

Centralized Admissions Coordinator

Location:

Primary location in Avon, CT. Must be willing to travel as needed.

Type:

• Full-Time Monday-Friday
• Rotating holidays and weekends
• Per Diem weekends

What You’ll Do:

• Review referrals from hospitals for all our skilled nursing facilities
• Collaborate with hospital case managers and internal teams to ensure smooth, timely admissions
• Make clinical and financial admission decisions that help patients transition to the right level of care
