Job summary

Pro Care by Abound Health is seeking a Central Intake Coordinator to join our Operations team. You will report to the Central Intake Supervisor and help move referrals from intake to activation, guiding members and caregivers through onboarding and documentation collection.

You will maintain accurate records in OTC, ClearCo, and other systems, coordinate with HR, Training, and Care Coordinators, and escalate barriers to timely service activation as needed.

Qualifications

• Associate's degree required, preferably in healthcare administration, business, human services, or a related field.
• Healthcare, human services, intake, onboarding, scheduling, or administrative experience preferred.

Responsibilities

• Receive, review, and track referrals from intake through service activation.
• Contact members, guardians, caregivers, and referral sources to collect required documentation and explain next steps.
• Create and maintain accurate member and caregiver records in OTC, ClearCo, and other approved systems.
• Coordinate with HR, Training, Care Coordinators, Recruiting, and Operations teams to facilitate onboarding.
• Track outstanding requirements, identify barriers to service activation, and elevate concerns to the Central Intake Supervisor as needed.
• Coordinate staffing needs for referrals that do not have an identified caregiver.
• Complete accurate handoffs to Care Coordinators and other departments to ensure continuity of care and services.
