CareCore Health is seeking an organized Central Intake Coordinator to support referrals and admissions across multiple skilled nursing facilities in the Cincinnati region. You will collaborate with Marketing, Business Development, and facility leadership to move referrals from initial review through admission.

This role requires strong coordination, attention to detail, and proficiency with electronic medical records and insurance verification.

Responsibilities

• Manage and process new referrals for multiple CareCore facilities.
• Verify insurance eligibility, coverage, network status, and authorization requirements.
• Initiate and track insurance authorizations through completion.
• Monitor referrals for updates, documentation, and potential barriers to admission.
• Ensure admission documentation (HENS/PASRR, Medicaid Pending) is completed.
• Prepare and maintain admission documentation in PCC.
• Communicate insurance findings and barriers to Marketing and facility teams.
• Prepare daily admission reports and communications.
• Cross-train across facilities and provide coverage as needed.
• Participate in holiday coverage rotation.

CareCore Health is seeking an organized, detail-oriented Central Intake Coordinator to support the referral and admissions process across multiple skilled nursing facilities in the Cincinnati Reigon.

This role is an important part of our centralized intake team and works closely with Business Development, Marketing, and facility leadership to ensure referrals move efficiently from initial review through admission.

Responsibilities

• Manage and process new referrals in GatewAI for multiple CareCore facilities

• Verify insurance eligibility, coverage, network status, and authorization requirements

• Initiate, track, and follow insurance authorizations through completion

• Monitor referrals for updates, documentation, and potential barriers to admission

• Ensure required admission documentation, including HENS/PASRR and Medicaid Pending requirements is completed

• Prepare and maintain accurate admission documentation in PointClickCare (PCC)

• Communicate insurance findings, missing information, and admission barriers to Marketing and facility teams

• Prepare daily admission reports and new admission communications

• Cross-train across CareCore facilities and provide team coverage as needed

• Participate in an assigned holiday coverage rotation

Qualifications

• Strong organizational and time-management skills

• Ability to manage multiple facilities, referrals, and competing priorities

• Excellent attention to detail and documentation accuracy

• Professional written and verbal communication skills

• Comfortable learning and working within referral, insurance, authorization, and electronic medical record systems

• Knowledge of healthcare insurance verification and authorization processes, or the ability to learn quickly

• Ability to work independently while collaborating with a centralized team

• Strong follow-through and ability to recognize and elevate barriers appropriately

Preferred Experience

• Experience in skilled nursing, post-acute care, admissions, intake, healthcare business development support, insurance verification, authorization, or a related healthcare setting

• Experience working with Medicare, Medicaid, Managed Care, and commercial insurance

• Experience with PointClickCare (PCC) and/or electronic referral management systems is strongly preferred
