Pro Care by Abound Health is seeking a Central Intake Coordinator to join our Operations team. You will report to the Central Intake Supervisor and help move referrals from intake to activation, guiding members and caregivers through onboarding and documentation collection.

You will maintain accurate records in OTC, ClearCo, and other systems, coordinate with HR, Training, and Care Coordinators, and escalate barriers to timely service activation as needed.

Formación

• Associate's degree required, preferably in healthcare administration, business, human services, or a related field.
• Healthcare, human services, intake, onboarding, scheduling, or administrative experience preferred.

Responsabilidades

• Complete accurate handoffs to Care Coordinators and other departments to ensure continuity of care and services.

Pro Care by Abound Health is seeking a Central Intake Coordinator to join our Operations team. In this role, you will report to the Central Intake Supervisor. The Central Intake Coordinator processes referrals and guides members and caregivers through onboarding to help members become active and staffed with caregivers. This role coordinates required documentation, follows up on incomplete onboarding steps, maintains accurate records in ClearCo and OTC, and works closely with internal teams to support a timely start of services.

• Proficiency with databases, technology platforms, and documentation systems.
• Ability to problem-solve, navigate barriers, and drive referrals to completion.
• Healthcare, human services, intake, onboarding, scheduling, or administrative experience preferred.

Education

• Associate's degree required, preferably in healthcare administration, business, human services, or a related field.

• The target compensation range for this position will be discussed during the interview process. Base pay is determined by factors such as experience, job-related skills, education, and work location.
