Qualifications

• Bachelor's degree preferred or equivalent combination of education and experience.
• Some college courses from an accredited college or university or equivalent education and experience.
• In lieu of higher education, one year of college equals one year of work experience and vice versa.

Responsibilities

• Spends 80% of time on patient facilitation to deliver exceptional patient and client experience.
• Support day-to-day execution of the medical model with clinicians to drive outcomes and case closure.
• Coordinate scheduling and staff with clinical leadership for efficient patient care and turnaround times.
• Develop a professional, collaborative environment promoting teamwork, quality, and patient safety.
