Job summary

Oak Street Health is seeking a Center Nurse in Bronx, NY to provide high-quality, protocol-driven care within a Value-Based Care model. You will perform clinical assessments, chronic condition management, and collaborate with an interdisciplinary team to reduce avoidable hospitalizations.

You will deliver education to patients and caregivers, conduct vaccines, manage medications, and participate in weekly transitions from hospital to clinic. Excellent communication and teamwork are essential.

Qualifications

• Active, unrestricted RN licensure in the state of residence (New York).
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• EMR experience.

Responsibilities

• Support patient assessment and triage to ensure appropriate level of care.
• Perform wound care and medication administration (including vaccines).
• Provide patient and caregiver education on chronic conditions and preventative care.
• Conduct medication reviews during nurse-led visits to ensure safety and adherence.
• Perform onsite testing (point-of-care labs, EKGs) and collect specimens as needed.
• Collaborate with providers to manage complex care plans and coordinate transitions from hospital to clinic.
• Utilize standardized protocols for medication management and renewals.
• Delegate tasks to medical assistants and phlebotomists to support patient flow.
